Inspector Notes
A Subsidy Health and Safety inspection was conducted on 5/31/2022 to review supplemental health and safety requirements. A total of 5 children were present with the provider. The children were observed transitioning from lunch to naptime. Programming, menus, emergency procedures, medications, and 4 children?s records, 1 staff record and 1 household member records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

1Evidence: There were 5 children present at the time of inspection and attendance had not been done yet for the day. After an interview with the provider, the attendance record was updated at approximately 12:04pm documenting the arrival of the 5 children between 7:30am and 8:00am today.
