Inspector Notes
An unannounced renewal inspection was conducted between the hours of 8:41 a.m. through 10:09 a.m. There were a total of 6 children present, ages 4 months, 10 months, 12 months, 21 months, 23 months and 2 years (20 points) with one Provider. A total of five children?s records, 1 staff file and 2 household member?s files were reviewed. The children were engaged in free play and watching a television show. There was an adequate supply of age appropriate materials and toys. The first aid kit, flashlight, emergency radio and fire extinguisher were observed. The emergency drill log, attendance and menu were also reviewed. The provider has current certification in CPR and First Aid. The provider reported there were no medications on site today. Not all areas of the home could be observed during the inspection, the provider stated the master bath was locked due to a leak. Areas of non-compliance are identified in the Violation Notice.

Violations
10Evidence: The record for household member #1 did not contain documentation of a completed sworn disclosure statement.
Evidence: The record for household member #2 did not contain documentation of the results of a search of central registry at the time of inspection.
Evidence: The record for household member #1 did not have documentation of tuberculosis testing or screening at the time of inspection.
Evidence: The last documented TB test for Staff #1 is dated 8/5/20
Evidence: At the time of the inspection the Licensing Inspector observed an area rug covering wood flooring in disrepair. One board was missing and several unsecured boards remained in the cavity covered by the area rug, causing a loss of balance when walked across.
Evidence: During the inspection the bottle warmer was plugged in with the power button on for approximately 45 minutes when not in use.
Evidence: A pressure mounted gate was installed at the bottom of the stairway to prevent access to stairs.
Evidence: At approximately 9:00 a.m. on 6/6/23 the attendance record did not contain documentation of five children in care. The attendance record dated 6/5/23 did not contain documentation of children in care, the provider confirmed there were children in care on this date.
Evidence:
1. Child #4, 2 years old, was observed seat belted in an infant swing. The child
was crying and leaning forward.
2. The manufacturer?s warning label on the base of the swing stated ?Stop using this product when the baby can climb?
Evidence: At the time of inspection there were six children present, ranging in age from 4 months to 2 years that totaled twenty points with one caregiver.
