Inspector Notes
An unannounced monitoring inspection was conducted 2/8/2024 between the hours of 12:55pm through 2:10pm. In addition, the provider submitted a request for modification to increase the capacity to 12. Zoning approval from Prince William County was observed. There were a total of 10 enrolled children and 1 resident child present (34 points) with the Provider and 1 assistant. A total of 6 children?s files, 2 staff files, and 1 household member?s files were reviewed. The children were observed napping or participating in quiet activities during the inspection. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. One new staff has been hired since the last inspection. The provider advised there was no medication on site today. There was an adequate supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.

4Evidence: Staff #2 (Date of Hire: 5/1/2023) did not have Virginia Child Abuse and Neglect Central Registry search results on file at the time of inspection.
Evidence: Staff #2 (Date of Hire: 5/1/2023) had only one reference check on file at the time of inspection.
Evidence: There were 10 enrolled children and 1 resident child present at the time of inspection. The current licensed capacity for the family day home is 8 children.
As explained, there was miss-understanding, Zoning was approved for 12 kids and I thought I have the approval for 12.
Based on observation and record review, the provider did not ensure that a caregiver does not exceed 16 points by using the supervision point system to determine if an additional caregiver is needed.
Evidence: 10 enrolled children and 1 resident children ranging in age from 5 months to 4 years old (34 points) were present with 2 caregivers at the time of inspection.
