An unannounced renewal inspection was conducted 6/12/2024 between the hours of 11:30am through 12:45pm. There were a total of 8 children present, ranging in age from 11 months to 5 years old (20 points), with the Provider and 1 assistant. A total of 8 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in various free play activities, eating lunch, and getting ready for nap time. Lunch served today consisted of milk, beef meatballs, apples, carrots, tomatoes, and rice. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an adequate supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on 7/2/2024. Please email me at morgan.bryson@doe.virginia.gov with any questions.
Violations
3Evidence: Three children under the age of two years were present at the time of inspection and there was no gate at the bottom of the stairs leading into the programming space where the children were playing.
Based on record review, the provider did not ensure that children's records contained all required information.
Evidence: Child #1's record was missing two emergency contact names, addresses, and phone numbers, the medical insurance policy number, and the parent's signed acknowledgement of the receipt of the information on the information for parents form.
Evidence: 6 children transitioned from free play activities to eating lunch without first washing their hands before beginning to eat.
