During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site 9:55am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 3 caregivers; the point total was 44. The inspector reviewed 7 children?s records and 6 caregiver/household members records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/4/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-140-B-3
Family Day Home Providers shall have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

The provider's CPR certification has been expired for over one month.
Plan of Correction: Started new CPR certification and will be getting my new CPR certificate in three days. Once obtained, will email you a copy for your reference.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver documentation of a negative Tuberculosis (TB) screening or test at least every two years or more frequently as recommended by a physician or the local health department.

Staff #2's updated TB screening was overdue by two weeks and has not yet been completed.
Plan of Correction: I've taken appointments for TB tests of my personals and we'll get everyone's TB test renewed/updated within 5 days.
Standard 8VAC20-800-40-A
The provider shall ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.

The provider's licensed capacity is 12 children. There were 13 enrolled children present throughout the inspection.
Plan of Correction: Acknowledged. *Redacted* is moving out and today was their last day, as they'll be starting pre-school soon.
New baby enrolled *Redacted* on May 27 is the replacement for *Redacted*.
Total kids receiving care is 12.
Standard 8VAC20-800-60-B
Repeat Violation

Children's records shall contain all required information.

Child #4 and Child #5's documentation of the required parental annual review of the child's emergency contact information was overdue by four months.
Plan of Correction: All kids records are updated and the two missing children records were updated and revised and up to date records are on file now for further access.
Standard 8VAC20-800-70-A
Before the child's first day of attendance, parents shall be provided required written information regarding the family day home's policies and procedures.

Child #3 has been enrolled for four months and did not have documentation on file that the parent's were provided the provider's policy regarding the administration of medication.
Plan of Correction: Acknowledged. Child No. 3's record updated and from now on, I'll make sure every parents receive the written policy and procedures in order to comply.
Standard 8VAC20-800-90-A
The provider shall obtain documentation that the child has been adequately immunized according to the requirements of the Code of Virginia and applicable State Board of Health regulations.

Child #7 has attended the program for two days and did not have immunizations on file.
Plan of Correction: Today during dismissal time, all parents (specifically Child #7) were advised once again to provide updated immunized records and parents are given 5 days until 06/03/2025 to provide up to date records.
