Repeat Violation

The provider shall obtain documentation signed by a licensed physician, the physician's designee, or an official of a local health department that includes dates immunizations were received for all children by date of enrollment.

Child #1 did not have immunization records on file that were signed by a physician, physician's designee, or an official of a local health department.
Plan of Correction: We maintained Child #1's immunization record at enrollment; however, the signature appeared only on the final page rather than on all required pages. This deficiency has been corrected by obtaining documentation signed by an authorized medical professional. The properly executed record is now on file and available for review, and our enrollment file checklist has been updated to ensure ongoing compliance.
