During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
A subsidy health and safety inspection (SHSI) was initiated on November 27, 2024 and completed on November 27, 2024. The inspection started at 10:30 am and concluded at 11:30 am. There were two children present and one caregivers; the point total was one. The inspector reviewed two children?s records and one caregiver records on-site.
This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Violations
2The family day home provides transportation to Child #1, who currently attends the family day home, but the child?s record does not contain permission to transport the child.
Caregivers shall complete a minimum of 16 hours of training annually. Caregiver #1 had not obtained any of the 16 required hours in 2023.
