Violations

2
Standard 8VAC20-780-340-D
Based on interview and record review, the center failed to ensure that at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.

Evidence:
1. Staff #3 (DOH 09/12/2023) and Staff #4 (DOH 10/16/2023) were working in the classroom together. The staff records revealed both Staff #3 and Staff #4 were hired as Assistant Teachers and did not have the experience or qualifications to be program leaders.
Plan of Correction: We will begin to look for more people with the 6 months experience and complete the 24 hours training within the 30 days.
Standard 8VAC20-780-350-K
Based on interviews, the center failed to ensure that an additional staff member shall always be available on-site to offer immediate assistance and shall be able to summon the additional staff member without leaving the room or area of the sleeping or resting children.

Evidence:
1. Staff interviews revealed that three of the five staff were interviewed and confirmed that during lunch/rest/nap time there is not an additional staff member on-site to offer help if they needed immediate assistance.
Plan of Correction: We are in the process of hiring two extra PT administrative people that can also step into the classroom or help throughout the day.
