During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and concluded on June 3, 2025. The on-site inspection began at 10:17 a.m. and ended at 12:00 p.m. The inspector reviewed compliance in the areas listed above. There were 32 children present and 8 staff. The program director was not on-site. The inspector reviewed 5 children?s records and 8 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 17, 2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Spray bottles of bleach water where unlocked on the sink in the infant, two's and four's classrooms. The spray bottles were in reach of children in the two's and four's classrooms.

Repeat Violation
Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. During the inspection, staff #1, staff # 2, staff #3, staff #4, staff #5, staff #6, and staff #7, who do not meet lead teacher qualifications, were working with groups of children without the presence of a lead teacher in each group.
Plan of Correction: Train staff and update status for each classroom.

Documentation shall be maintained of emergency evacuation that includes all the required information. Documentation of the fire drill for May 2025 did not contain the identity of the person conducting the drill, the method of use for notification of the drill, the number of staff participating, the number of children participating, any special conditions simulated, the time it took to complete the drill, problems encountered, and the weather conditions.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and contain all the required information. An incident report for child #7 did not contain the time the parent was notified. An incident report for child #8 did not contain the time the parent was notified and the future action to prevent recurrence. An incident report for child #8 did not contain the date and time the parent was notified and how the parent was notified.

The record of each child on formula shall contain the brand of formula. There was no documentation of the brand of formula in the records of children #2, #3, and #4.
