Inspector Notes

A monitoring inspection was initiated on 05/20/2024 and concluded on 05/20/2024 from 9:30 AM to 11:20 AM. There were 52 children present, ranging in ages from infants to five, with 15 staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the five staff?s records were reviewed/updated.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on observation, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In the older toddler classroom, the bottom cabinet was left unlocked containing Drano, bleach and Clorox wipes. The label on the Drano stated "keep out of reach of children," and "danger." The label on the Clorox bleach stated "keep out of reach of children" and "warning." The label on the Clorox wipes stated "keep out of reach of children" and "caution".
2. Administration agreed that the bottom cabinet was unlocked.
