Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on April 15, 2021 and concluded on April 16, 2021. The provider was contacted by telephone to initiate the inspection. There were four children present and two caregivers. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children?s records and one caregivers record submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of two children's records, the provider did not obtain all required information for one child's record.

Evidence:

The record of child #1, enrolled 09/08/2020, did not contain the name, address and phone number of a second person to contact in an emergency if the parent cannot be reached.
Plan of Correction: Per the provider: I will have the parent complete the missing information in the child's record.
