Inspector Notes

An on-site follow-up monitoring inspection was conducted on October 28, 2022. The licensing inspector was onsite from approximately 1:10 pm until 3:15pm. The substitute provider assisted with the inspection until the arrival of the provider at approximately 2:50 pm. There were nine children present, ranging in ages from 14 months to four years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records, three caregiver
records and one household member record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-170-A
Based on review of three caregiver records, the provider did not obtain documentation of a negative tuberculosis screening for one caregiver at the time of hire.

Evidence:

The record of caregiver #2, hired 09/12/2022, contained documentation of a tuberculosis screening dated 09/14/2022, exceeding the date of hire.
Plan of Correction: Per the provider: I will obtain tuberculosis screenings at the time of hire.
Standard 8VAC20-800-180-A
Based on review of caregiver and household member records, the provider did not obtain a current report of tuberculosis screening every two years from the date of the first screening.

Evidence:

1. The record of caregiver #3 contained documentation of a tuberculosis screening dated 7/21/2020, exceeding two years.
2. The record of household member #1 contained documentation of a tuberculosis screening dated 07/20/2020, exceeding two years
Plan of Correction: Per the provider: I will have the tuberculosis screening updated and will maintain them every two years.
Standard 8VAC20-800-430-A
Based on observation the provider did not ensure children under two years of age and children over two years of age who are not developmentally ready to climb or descend stairs without supervision did not have access to stairs.

Evidence:

On October 28, 2022, the gate at the bottom of the staircase in the childcare room was open creating access to the stairs. Child #4, 14 months in age was in attendance.
Plan of Correction: The substitute provider closed the gate at the bottom of the stairs and ensure it was secured while the inspector was on site. Per the provider: the gate will be kept closed while children are in care.
