Inspector Notes
A monitoring inspection was conducted on 07/17/2023 from11:00 am to 1:45 pm. There were eleven children in care, ranging in ages from 20 months to 7 years with three caregivers supervising for a total of 22 points.
The inspector reviewed compliance in the areas of administration, care of children, physical plant, physical equipment and environment, staffing and supervision, programming, special care and emergencies and nutrition.
A total of five child records, and five caregiver records were reviewed. The record for one household member was reviewed. The children were observed playing outside and participating in an art activity. Handwashing, diapering, lunch time, and napping was observed. A variety of age appropriate toys, equipment and supplies were available to the children.
The caregiver's interactions with the children were positive. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not
acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

1.The record for Caregiver #4 (date of employment: 5/9/2022) did not document 16 hours of annual training.
2. During an interview with the provider, it was acknowledged that the record for caregiver #4 did not document the required 16 hours of annual training.

1.The record for Child #3, did not document the address of the two designated people to call in an emergency if a parent cannot be reached.
