8VAC20-790 Subsidy Program Vendor Requirements for Family Day Homes

Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on July 18, 2024 with the provider and two assistants. The on-site inspection began at 1:45pm and ended at 4:10pm. There were 6 children present with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the provider.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on review of caregiver records, the provider did not ensure all caregivers have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.

Evidence:
1. The record for caregiver #3 contained a CPR certification that expired on 2/12/2024.
2. The provider acknowledged not having a current CPR certification for caregiver #3.
Plan of Correction: Staff will take certification.
