Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 6/8/2021 and concluded on 6/8/2021. The director was contacted by telephone to initiate the inspection. There were 56 children present and 16 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review, the center did not obtain a physical examination for each enrolled child within the first month of attendance.

Evidence:
Child#1's (FDA: 5/3/2021) file does not contain documentation of their physical examination.
Plan of Correction: Have spoken to family and issued a deadline to get this.
Standard 63.2(17)-1720.1-B-4
Based on record review and interview, the center did not obtain a sex offender registry check from a state in which the applicant has resided in the preceding five years prior to employment.

Evidence:
Staff#1's sex offender results were obtained on 6/7/2021. The staff member's sex offender results were obtained after their employment date of 5/10/2021.
Plan of Correction: Have established a method to better track this and put in place on 6/8/2021.
