Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.
A monitoring inspection was initiated on 12/08/2021 and concluded on 12/08/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 117 children present, with 27 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on staff record review, out-of-state background check results were not obtained for staff members that have resided outside the state of Virginia within the last 5 years.

Evidence:
Staff #1 (DOH: 08/16/2021) has resided outside the state of Virginia within the past 5 years, their records contained documentation of out-of-state child abuse and neglect results and sex offender search results that were completed in 2018 by a prior employer.
Staff #2 (DOH: 08/18/2021) has resided outside the state of Virginia within the past 5 years, their records did not have documentation that an out-of-state sex offender search was completed prior to the staff person's first day of employment.
Plan of Correction: Sex offender completed on 12/9/2021. Child Abuse/Neglect submitted 12/16/2021 - awaiting results.

Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child #1's record contains an immunization record that was last updated on 3/17/2021.
Plan of Correction: Updated and submitted to inspector.

Based on review of records and interview, the facility's annual fire inspection report has expired.

Evidence:
The date of the most recent fire inspection conducted by the fire official was 12/02/2020.
Plan of Correction: Have emailed, called and submitted online request for inspection. Still awaiting response.
