Inspector Notes
An unannounced monitoring inspection and a self-reported incident investigation related to behavioral guidance and programs were conducted today from 10:30am-2:25pm. There were 122 children (infants-5yrs.) directly supervised by 30 staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, participating in circle time, doing puzzles, playing with Legos and small manipulative toys, playing outside, and eating lunch. There was an adequate supply of materials for the children. The center was clean and organized. Interviews with staff were conducted and documentation and records related the incident were reviewed. The evidence gathered during the investigation did not support s the self-report of non-compliance with standards or law, there were no violations cited related to the self-reported incident; however, areas of non-compliance not related to the self-reported incident are identified on the Violation Notice. Staff #5's record contains Fingerprint results that were dated 3/3/2022, the staff member's first day of employment was documented as 2/15/2022.
Evidence:
Staff #1's (DOH: 4/28/2022) records indicate that they have resided outside the state of Virginia within the past 5 years, their records did not include documentation that out-of-state criminal history results were obtained before their first day of employment.

1. Child #2's record contains immunization documentation that was last updated on 9/23/2021.
2. Child #3's record contains immunization documentation that was last updated on 3/12/2021.
3. Child #4's record contains immunization documentation that was last updated on 10/28/2021.
Evidence:
1. The medication authorization for child #6 expired on 4/25/2022, but the medication was still at the center.
2. The medication for child #6 had been present in the center for more than 14 days since the expiration of the written authorization.
