Inspector Notes
An unannounced renewal inspection was conducted today from 12:00pm-2:15pm. There were 128 children (infants-5yrs.) directly supervised by 28 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, napping, playing outside, and eating lunch. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Staff #4's (DOH: 5/15/2023) record did not include a out-of-state criminal record check.
Evidence:
The allergy list did not include a child currently attending the program that has a dietary restriction. The child's record confirmed that they have an intolerance to dairy products and soy products.
Evidence:
1.Staff #1 and Staff #5's records did not contain documentation that two or more references were checked before employment.
2. Staff #4's record was missing one of two required references.
