Inspector Notes

A monitoring inspection was conducted today between 11:15am and 2:00pm. There were 110 children present, with 31 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. The children were observed playing in the gym, washing hands, and eating lunch. A total of 10 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. REPEAT VIOLATION
Based on review of staff records, the child day center did not obtain out of state background checks for staff who have lived outside the date of Virginia within the past five years.
Evidence:
Staff #1, first day of employment 9/18/2023, has lived in Georgia and North Carolina within the past five years. The child day center did not follow up on the out of state central registry checks for Georgia and North Carolina after 45 days.
Plan of Correction: I followed up today with North Carolina and Georgie to see where the results are. When they are received I will send the results to my inspector and place a copy of them in the staff record. We will follow up on all background checks not received on time.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence
A bleach and water bottle and an ant farm, (keep out of reach of children) were accessible to children in an open closet in the Get Set classroom.
Plan of Correction: Staff will make sure the closet is always locked. Corrected during the inspection.
Standard 8VAC20-780-440-B
Based on observation, cots were not identified for individual use.
Evidence
Cots located in the Toddler classroom were not all identified for individual use.
Plan of Correction: I will make sure all of the cots are labeled for individual use. Some cots didn't get labeled due to many children changing classrooms today.
Standard 8VAC20-780-560-G
Based on observation, when food was brought from home, the date was not clearly identifiable.
Evidence
Lunch boxes brought from home were not all labeled with the current date.
Plan of Correction: We will re-train to make sure food from home is always labeled.
Standard 8VAC20-780-60-A
Based on review of children's records, the center did not maintain a complete record for each child enrolled int he center.
Evidence
The center did not have documentation of parental review of the records for child #1 and #2.
Plan of Correction: We will review child files with a parent and have them sign and date when they review and make any changes.
