During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 11/4/2024, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 1:35pm. The inspector reviewed compliance in the areas listed above. There were 125 children present and 27 staff. The inspector reviewed 10 children?s records and 5 staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Violations

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Immunizations for Child #1 were obtained approximately one month after attending the center.
Plan of Correction: The center will ensure immunizations are obtained by the center prior to a child's first day of attendance.
Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Child #1's physical was not obtained for 6 months after first day of attending.
Plan of Correction: The center will ensure that a physical exam is obtained by the center within 30 days of child's attendance.
Standard 8VAC20-780-260-B
After the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for water supply and sewage disposal system.

The center did not have documentation of or knowledge that an Environmental Health and Safety Inspection is to be done annually in addition to the food service inspection.
Plan of Correction: Center staff contacted Department of Health to request Environmental Health and safety inspection to include water and sewage disposal and will continue to request it annually.
Standard 8VAC20-780-60-A
REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for each child
enrolled which shall contain name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Child #1's record did not have address for 2 emergency contacts; Child #3's record did not have address for 1 emergency contact.
Plan of Correction: The center will follow up with parent to obtain emergency contact information and review children's record for complete information routinely.
