Inspector Notes

An unannounced monitoring inspection was initiated 8/26/2025 and completed on 8/27/2025 in response to a self-report received by the licensing office on 8/26/2025 relating to behavior and guidance. The inspector reviewed 3 children?s records and 2 staff records on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/11/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 8/21/2025 Staff#1 stated, that child #1 was observed with hands on another child's neck. Staff separated the children and when child got up from chair, staff #1 "grabbed" child to sit down.
Staff #1 was observed by staff #2 grabbing child#1 by the shoulders, while sitting down.
Plan of Correction: Additional training for all staff and observation and coaching to begin 9/17/2025.
