Inspector Notes

An unannounced monitoring inspection was initiated on 3/27/2026 and completed on 4/6/2026 in response to a self-report received by the licensing office on 3/26/2026 relating to staffing and supervision. The inspector reviewed 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/14/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 3/26/2026 a 3 1/2 yo child was found coming out of bathroom by a parent, without staff present. The class had left the classroom and the building.
Plan of Correction: Name to face, threshold count and retrain staff on all areas.

Children under 10 years of age always shall be within actual sight and sound supervision of
staff.

On 3/26/2026 at approximately 4:15 PM, staff exited classroom and building with children and a 3yo child from that class was found in the classroom by a parent at approximately 4:18PM.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's
activities, services, and facilities are maintained in compliance with the center's own
policies and procedures.
The center?s policy/protocol regarding knowing where children are verbally and written
tracking/name-to-face forms, for the Bridge classroom was not completed and a child was left unattended in classroom for approximately 3 minutes, while class was outside of building.
Plan of Correction: Use device or paper attendance ensuring name to face and threshold counts are conducted at every entry.
