Inspector Notes

An unannounced monitoring inspection was conducted on January 5, 2023, to determine the centers compliance with licensing standards. The inspection was initiated at approximately 9:50 AM and concluded at 11:30 AM. There were18 children in care with 5 staff supervising. Children were observed playing with age appropriate materials, and watching movies. The inspector inspected the inside and outside play areas, restrooms, and kitchen. No transportation is provided at this time. All required postings were reviewed and found to be complete. The center's first aid kit and non-medical emergency supplies were inspected and found complete. A total of 5 children?s records and 5 staff records were reviewed. Based on review of staff records, the center failed to obtain the results of a follow-up tuberculosis screening every two years after the first initial screening or testing.
Evidence:
1. The record for Staff #3 did not contain documentation of an updated TB test or screening. Last documented update was 12/15/2020.
2. Staff verified that the record did not contain documentation of an updated TB test or screening.
Plan of Correction: The director will schedule an appointment.
Standard 8VAC20-780-260-B
Based on a review of the most recent health inspection, the center did not ensure that annual approval from the health department shall be provided for meeting requirements for water supply, sewage disposal system and food service.
Evidence:
1. Documentation of the most recent health inspection is dated 12/31/2022.
2. The director confirmed that the most recent health inspection is expired.

Based upon measurements and staff interview, the facility has not ensured that each restroom used by the children has sinks with water that does not exceed 120 degrees F. Evidence:
1. Hot water temperature in the bathroom sink located in the infant room used by the children in care registered 129 degrees F.
2 Hot water temperature in the bathroom sink located in the toddler classroom used by the children in care registered at 130 degrees F.
Plan of Correction: Maintenance man will be contacted.

Based on review of the drill log and interview, the center failed to ensure a minimum of two shelter-in-place practice drills were completed each year.
Evidence:
1. There was no documentation of a shelter in place drill.
2. Per the director, there were no shelter-in-place drill conducted during the 2022 year.

Based on observation, the center failed to ensure that all prepared infant formula was labeled with the date.
Evidence: I
1. In the infant room there were four bottles with prepared formula that were not labeled with the date.
2. Staff verified that the bottles were not labeled with the date.
