During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on October 1, 2025. The on-site inspection began at 10:20am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 7 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included a tour of the facility, document review, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1. The carpet in the infant room shows several signs of stains and/or soiled marks. The carpet appears darkened with a buildup of grime over years of use.

2. In the infant room, two crib mattresses were found to be ripped and torn exposing the interior of the mattresses.

3. The changing table in the infant room had crumbs and dirt build up around the edges of the changing pad.
Plan of Correction: New carpet coming asap. New cribs mattresses in the attic.

When centers choose to provide meals or snacks a menu listing foods to be served for meals and snacks during the current one-week period shall list any substituted food.

The menu posted had sloppy joe listed to be served for lunch on 10/1/2025. The menu was not updated with the substitute food to show that chicken would be served instead for lunch on 10/1/2025.
