Standard 22.1-289.035-B-2
REPEAT VIOLATION The center must complete fingerprint-based criminal history determination letter prior to 1st day of employment. Staff #2 has been employed for three months and does has not obtained a fingerprint-based criminal history determination letter.
Plan of Correction: The fingerprints for staff #2 will be completed and processed within ten days. The results will be sent to the licensing inspector. In the future this process will be completed prior to hire.

REPEAT VIOLATION The center must obtained a central registry (CPS) record check prior to 1st day of employment or requested for provisional hire. Staff #2 has been employed for three months and does has not obtained a CPS record check. The request was sent after hire.
Plan of Correction: The CPS for staff #2 will be processed within ten days. The results will be sent to the licensing inspector. In the future this process will be completed prior to hire.
Standard 22.1-289.049-A
Upon enrollment of a child in a regulated child day program, such child day program shall require information from the person enrolling the child regarding previous child day care and schools attended by the child. The regulated child day program shall also require that the person enrolling the child present the regulated child day program with the proof of the child's identity and age. The center has not requested this information for any child in care.
Plan of Correction: This information will be requested from parents for all children. In the future this information will be obtained at registration.
