Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a virtual tour of the program.

A renewal inspection was initiated on July 23, 2021, and concluded on August 3, 2021, approximately 10:09 am to 12:01pm. The director was contacted by telephone and a virtual inspection was conducted. There were 14 children present, ranging in ages from infants to 5 years old, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on children records, 1 out 3, the center did not obtain documentation that each have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

Evidence: Child #3 (date of enrollment 3/22/2021) did not have a physical examination in file available for review during the inspection.
Plan of Correction: Called Parent and they will bring it Thursday from the Doctor's

Based on observation, the center did not have an emergency evacuation and shelter-in-place procedures posted in a location conspicuous to staff and children on each floor of each building.
Evidence: The emergency evacuation and shelter-in-place procedures were not posted and available for review during the inspection.
