During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on July 9, 2025 and completed on August 21, 2025 with a local agency in response to a self-report received by the licensing office on July 2, 2025 relating to care and protection and forbidden actions. The inspector reviewed 5 children?s records and 6 staff records on-site on July 9, 2025. This inspection included document review, tour of the facility,interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 28, 2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
Systemic Deficiency

The center must request a search of the central registry prior to the employee's first day of employment. Staff 2, who had been employed for two weeks, did not have a central registry search requested.
Plan of Correction: Before staff start working in the center, they get a sheet of paper of what needs to be completed before working. So that their staff folder is complete. They meet for the orientation and do the central registry with the center director before they leave out of the orientation. We make sure that we go over the sworn statement and make sure that if we need to request an out of state, we do that during orientation as well. All documentation and training are done during the orientation and documented so that it can be put in the file for the school. Once the file is complete the center director scans the whole file to herself to have it on email as well on paper, so nothing gets lost in the process.

Staff employed by a child day program who have reason to suspect that a child is abused or neglected shall report the matter immediately to the local department's toll-free child abuse and neglect hotline. The center was notified by a staff that Staff 1 was seen grabbing Child 1 by the arms and failed to report it.
Plan of Correction: Staff take training on child abuse and neglect. Teachers before they start working will be required to take child abuse and neglect training before working in the center. During every training that the school does we go over child abuse and neglect if they see then we act for the matter.
Standard 8VAC20-770-70-A
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff 2, who has been actively working at the center for two weeks, did not have any completed reference verifications on file.
Plan of Correction: During orientation the center director will make sure to receive two references to call prior to working at the center. We ask the teachers to list two references that have worked with the teacher with their phone number, first and last name and where they have worked with that person before. Then the same day the center director will call and talk to the reference and ask them the reference questions to put in their file.

When staff are supervising children, they shall always ensure their care, protection, and guidance. Staff 1 rough handled Child 1 (age 2) around the week of June 23, 2025.
Plan of Correction: Administration will be walking around more frequently to ensure staff and children are well taken care of. Teachers also get training in the supervision of the children on the playground and in the classroom. Teachers are required to sign and date the form to ensure that they understand that supervision of the children is important while being in their care.

Systemic Deficiency

The center is required ensuring that the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center's policy and procedurs were not followed for mandatory reporting requirements and behavorial guidance.
Plan of Correction: the center had a meeting with teachers talking about if they see something with a teacher and a child that they must report it. Failing to report anything involving a child and a teacher will cause a write up or termination. Teachers have another meeting on August 29th to go over all the standards for the center. Teachers are required to sign a behavioral and reporting guidance during the meeting to ensure they understand the information and the importance.

The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child, restricting movement through binding or tying, forcing a child to assume an uncomfortable position, or exercise as punishment. Around the week of June 23, 2025, Child 1 sat on the cot crying during naptime. Staff 1 grabbed Child 1 by the arms to keep child from moving off the cot.
Plan of Correction: Teachers now get on how to handle children in they don't want ot sleep on thier cot. Teachers before they go into the classroom get training on appropriate touch to the children. Teachers also get a copy fo the orientation to have on hand to go over information as needed as well.
