22.1 Early Childhood Care and Education
During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 9, 2025 and completed on October 17, 2025, as a part of the provisional licensure period. The on-site inspection began at 10:15 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 9 staff. The inspector reviewed no children?s records and 3 staff records on-site on October 9, 2025.
This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 24, 2025. A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 7/09/2025 and the provisional license, were not posted .
Plan of Correction: Ms. Christina was not aware that the summary report need to be posted to clearly see it was posted behind the assistant director's desk. Now that Ms Christina is aware of the summary report needs to be in view for parents to see. It is now posted for parents to see in the center.
Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children. The screening for Staff #1 was not completed within 30 days of hire.
Plan of Correction: Once hired the employee will be told that they have 30 days from the start date to get a TB test done. During the interview, they also will be told that once hired they will need to get a tb test done within 30 dyays.

Repeat Violation
When staff are supervising children, they shall always ensure their care, protection, and guidance. On 10/09/2025, an infant child was sitting in a infant seat on the playground during the same time toddler and two year old children were running with push toys on the playground.
Plan of Correction: Teachers in each classroom have their outside time posted on the wall and outside the classroom for parents to see. Teachers had a meeting the admin and talked about ratio/outside time throughout the day. Teachers are now aware to only go outside on your time that is posted on the wall unless by the admin team. It teachers need to do anything in the center by either changing diapers or using the bathroom then they know to use their walkie talkies.

The required ratio for children ages Birth to 16 month is one staff per four children. There were four staff observed supervising 24 children (youngest child16 months old) from approximately 10:15 am to 10:30 am.
Plan of Correction: There was a meeting on Monday October 13th at 6:00 pm to Talk to staff and have a demonstration on ratio with all staff. Teaches learned and did multiple demonstrations and talked and had questions on rations inside the classroom and outside the classrooms. Teachers now know that it is mandatory to use walkie talkies if they need something, bathroom break, to change a diaper, etc.

Infants shall be protected from older children. During infant outdoor playground time, there were 9 infants on the playground with 5 toddler's and 11 two year old children.
Plan of Correction: Teachers have their outside time posted in the classroom and on their tablets also they now have time for going outside time and coming back in time. Teachers are encouraged to use their walkie talkies if another classroom is outside during their time. Admin now sets times in office to check and see if classes are outside or inside during their time they're supposed to go outside or come in .

Systemic Deficiency
Repeat Violation
The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with these standards, the standards own policies and procedures that are required by these standards, and terms of the current license issued by the department. The center's daily activity schedule for the infant, toddlers and two's classroom, safety procedures, and supervision policy were not followed. The policy and procedures not being followed on the playground at the same time resulting in staff to child ratio not being met.
Plan of Correction: Teachers got more training on ratios, and center activities. Teachers are now aware of what the plan for each classroom is and now have plans of how things work in the classroom. Teachers cine it admin if they have any questions about the classroom or if they have any questions about the classroom or if they need help with anything. Teachers have an updated schedule of what should be going on in the classroom with students.
Standard 8VAC20-780-70
Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment. Staff #1, Staff #2 and Staff #3 who have been actively working at the center for the past 2 months, did not have completed reference verifications on file with date of contact, signature of person who completed the reference check, and the relationship of reference for Staff #1, Staff #2, and Staff #3.
Plan of Correction: Before potential employees come into the center to work they need to give 2 references during the interview that the center director or assistant director or assistant director can call before any employment with the company. Once the interview is over the potential new hire leaves with a check list of items that need to be completed and sent back via email to the center director before employment.
