Inspector Notes

An unannounced monitoring inspection was conducted on November 29, 2023, from 1:57pm to 3:21pm with the provider. There were three daycare children and one residential child, ages 13 months, 18 months, 2, and 4 years old, were in the care of one caregiver for a total of 11 points. The children were observed during nap time. Three children?s records were reviewed. The provider?s and one household member?s records were reviewed and in compliance. There were no medications on site to be reviewed. The provider has current certification in CPR and First Aid. The first aid kit, flashlight, and battery-operated radio were observed. The most recent emergency escape drill was documented as 11/6/2023. The most recent shelter-in-place was documented as 7/8/2023.

Based on a review of three children's records, the provider did not obtained documentation of a physical examination for all children enrolled within 30 days after the first day of attendance. Evidence: Child A, child B, and child C's records did not have documentation of a physical examination. The children's start date was 7/24/2023.
