During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 6/5/2025. The on-site inspection began at 9:05am and ended at 10:05am. The inspector reviewed compliance in the areas listed above. There were 6 children present and 2 caregivers. The inspector reviewed 3 children?s records and 2 caregiver/household member(s) records on-site.
This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/18/2025. A POC submitted after this date will not appear on the public website.
Violations
4The was a tub of disinfectant wipes Labeled ?Keep out of reach of children? on a low shelf on the diaper changing table, accessible to children.
Child A (14 months) was observed in a crib that had no covering on the mattress.
The record for Child B, enrolled for ten months, was missing one name, address and telephone number for designated persons to contact in case of an emergency if the parent cannot be reached.
Provider did not conduct any shelter-in-place drills from May 2024-May 2025.
