A subsidy health and safety (SHSI) inspection was conducted on August 23, 2022 from 10:44 A.M.-1:45 P.M. There were eight children present with three caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of caregiver records, the provider failed to ensure that all caregivers completed the Virginia Preservice Training within 90 days of becoming a subsidy provider.
Evidence:
1. The provider became a subsidy participant in July of 2021. Caregiver #2 has not completed the Virginia Preservice Training. It was required to be completed by the end of October 2021.
