During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on August 28, 2025 and completed on August 28, 2025. The on-site inspection began at 10:20 a.m. and ended at 11:20 p.m. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was ten. The inspector reviewed five children?s records. No updates were needed at this time. Two caregiver/household member(s) records were reviewed on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for the adult household member was not completed and was overdue by five months.
Plan of Correction: An appointment was made for the adult household member for September 3, 2025. In the future the provider will ensure that all required people update TB screenings every two years.
