All adult household members must submit to fingerprint background checks. A household member turned 18 three months prior and has not submitted fingerprint result.

The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Child #1 and child #2 have no physical on record and started five months prior.
