Inspector Notes

This renewal inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact and remote virtual application as well as an onsite visit to the center

A mandated renewal inspection was initiated on 9/24/2021 and completed on 09/27/2021. There were 31 children in attendance at the time of the inspection. A code compliance inspection was conducted at the time of this inspection, as well as a review of regulations for child and staff information ( files), policies and procedures, emergency preparedness planning and drills and a tour of child care areas. Also confirmed floor plan data received at initial application.

Information gathered during the inspection determined a violation with applicable code or regulations. Detailed in attached Violation Notice.

Based upon observation, areas and or equipment were not maintained in good condition.
Evidence: Multiple pieces of play equipment on outside play grounds were found with broken pieces or cracked materials.
2 ride on cars with no steering wheels,2 wood benches with broken wood panels, stationary equipment with rusted or peeling paint
