Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy complaint inspection was initiated on 03/16/2021 and concluded on 3/30/2021. A subsidy complaint incident was received by the department regarding allegations in the areas of Special Care Provisions and Emergencies. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with subsidy regulations, and violation was issued.
Violations
1Evidence: 1. On March 10, 2021, a child was injured at the center and taken for outside medical treatment. The vendor failed to inform the department of the injury within the required time frame. 2. The center director confirmed a child was injured at the center and received outside medical treatment. 3.The center director submitted the notification of injury after complaint investigation was initiated on March 16, 2021.
