Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 03/16/2021 and concluded on 04/02/2021. A complaint was received by the department regarding allegations in the areas of Staffing and Supervision and Special Care Provision & Emergencies . The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with Staffing and Supervision and violation was issued. Any violations identified during the course of the investigation can be found on the violation notice.
Violations
1Evidence:
1-The center's video footage of March 10, 2021 showed child #1 , child #2 and child #3 observed running in the classroom pushing and hitting each other. Child #1 was pushed by child #2 into the sink and was observed holding his head walking away from the sink. The exchange between the children lasted for approximately 26 seconds. Video footage shows Staff #1 came over after child #1 was injured. Staff #2 provided first aid to child#1. Child#1 had a cut on his right eye brow that required medical treatment and three stitches.
2-A serious injury was documented on March 10, 2021 at 3:09 for child #1. Notification was sent to parent at 3:25pm through the parent engagement app. Parent arrived at approximately 4:15pm she was not aware of the parent engagement notification of child #1 `s injury. The center director confirmed the notification had only been sent though the parent engagement app and that she did not try to call the parent or notify additional emergency contacts.
2-The center will continue to contact the parent and/or emergency contact until confirmation. The center will notify through engagement app, text, phone call to parent or emergency contact and email until a person confirms the notification was received.
