Inspector Notes

An unannounced complaint inspection was initiated on July 11, 2023 and completed on September 1, 2023 regarding concerns of physical plant, staff qualification and training ,staffing and supervision and programs. There were observations in the classrooms, interviews with staff, a review of records/documentation. The preponderance of the evidence gathered during the investigation does support the allegation related to staff training and qualifications therefore, the complaint is determined to be valid. There was a violation not reported but founded. The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Based on staff interview and documentation, the licensee did not ensure children under 10 years of age always shall be with in actual sight and sound supervision of staff.

Evidence:
During inspection on July 11, 2023 , Staff #1 stated child was left on the playground.
Staff #1 stated she came in throught the playground door while kids were outside playing. Staff #1 was met in the hallway by parent who saw a child on the playground and did not see staff. Staff #1 immediatley went outside and brought the child#1 in the center. Staff #1 said it was less than a minute from the time she left the playground and kids were on the playground. Staff #1 confirmed the child#1 was alone.

Documentation provided by the center description of incident states on April 18 parent states that there was a child outside and she didn't see any teachers. Staff #1 went outside and there was a child outside.
Plan of Correction: Staff working with the age group was immediatley release on the day of the incident.
Reviewed with staff sight and sound supervision procedures.

Based on record review, the licensee did not enusure current certification in first aid.

Evidence:
Staff #3 and Staff #4 did not have a current First Aid Ceritfication and they work alone with children.
Plan of Correction: Staff #3 and staff #4 completed the First Aid.
The center will check more often in records to make sure certifications are updated.
