Inspector Notes

An unannounced inspection was initiated on 3-10-2025 and completed on 6-4-2025 with a local agencies, in response to a complaint received by the licensing office on 3-10-2025 relating to allegations of Administration, Programs, Special Care Provisions/ Emergencies and Special Services. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, violations that were not reported were found related to Programs and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6-11-2025. A POC submitted after this date will not appear on the public website.

The following acts are forbidden: physical punishment, striking a child, roughly handling or shaking a child. On 3-7-2025,.Staff #1roughly handled Child #1, when they grabbed Child #1 and pulled the child towards themself. Then Staff #1 grabbed the face of Child #1 under the chin and around their cheeks. Staff #1 then turned Child #1 around by the upper (left ) shoulder so that they faced away from Staff #1 and then lowered Child #1 to the ground and then took their pants off. Staff #1 grabbed Child #1 by the upper left arm and shoulder and stood them up by pulling their arm and shoulder. Child #1 sustained bruising on their arm as verified by a physician.
