Inspector Notes

An unannounced inspection was initiated on 6-2-2025 and completed on 6-4-2025, in response to a complaint received by the licensing office on 5-21-2025 relating to allegations of Administration,Special Care Provisions/ Emergencies and Special Services. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violation is documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6-11-
2025. A POC submitted after this date will not appear on the public website.

When centers choose to provide meals or snacks,the following shall apply:
1. Centers shall follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
2. Children shall be allowed second helpings of food listed in the USDA's child and adult care meal patterns.

On June 2, the lunch served to children ages 3-5, did not meet the age-appropriate nutritional requirements regarding portion sizes and second helpings were not available when requested by several children. During the month of May 2025, the breakfasts that were offered did not always meet the age-appropriate nutritional requirements and on more than one occasion only one component out of the three required was offered.
Plan of Correction: We will be adding additional hands on training for both the cook and staff to ensure they have a clear understanding of proper food measurements and required meal components
