Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A non-mandated monitoring inspection, resulting from an Intensive Plan of Correction, was conducted on 02/03/2022. The provider was contacted by Zoom and a virtual inspection was conducted. There were 21 children, ages 5 months- 4 years, and 5 staff members present during the inspection. The inspector reviewed compliance in the areas of administration and staff qualifications and trainings. A total of 3 children?s records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to ensure that an employee must not be employed until the agency has the person?s completed sworn statement or affirmation.

Evidence: The record for Staff 5 had a documented hire date of 01/24/2022; there was not a sworn statement available for review during the 02/03/2022 inspection.
Plan of Correction: Will get it completed. Will make an employment packet and use a checklist.

Based on record review and discussion with staff, the center failed to ensure that each staff record contains all of the elements as required by the standards.

Evidence:
1. The record for Staff 1 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment.
2. The record for Staff 2 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment.
3. The record for Staff 3 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment and health information required by 8VAC20-780-160.
4. The record for Staff 4 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment and health information required by 8VAC20-780-160.
