Inspector Notes
An unannounced monitoring inspection was conducted on 03/13/2023. There were 32 children, ages 2 months-9 years, and 8 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. The children were observed during breakfast, free choice activities, and participating in learning activities; infants were observed eating and sleeping on demand. A total of 5 children?s records and 5 staff records were reviewed. The inspector discussed the following with the staff: infant nutrition and training. The inspector arrived for the inspection at 8: 25 am and departed at 11:00 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
12Evidence: The record for Child 5 contained a documented first date of attendance of 02/15/2023. There was not an immunization record in the record for Child 5 available for review during the inspection. Child 5 was observed in care during the inspection.
Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children and the documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. The record for Staff 3 contained a documented hire date of 02/27/2023. The date on the tuberculosis screening was 03/20/2023. The tuberculosis screening was not completed within the last 30 calendar days of the date of employment.
2. The record for Staff 5 contained a documented hire date of 12/05/2022. There was not documentation of a negative tuberculosis screening available for review.
Evidence:
1. The record for Staff 1 contained a documented hire date of 11/03/2022. Staff 1 has not completed the Virginia Department of Education sponsored orientation course as of 03/13/2023.
2. The record for Staff 5 contained a documented hire date 12/05/2022. Staff 5 has not completed the Virginia Department of Education sponsored orientation course as of 03/13/2023.
Evidence: Child 3 was observed walking alone down the hallway from the Pre-K Classroom. Child 3 turned right around a corner, walked down the hallway of the Toddler/Infant Rooms, knocked on the door of the Toddler Room to get supplies from the staff in that class. Child 3 was then observed walking back to the Pre-K Classroom. Child 3 was not within actual sight and sound supervision of staff while walking on the Infant/Toddler Class hallway. Child 3 was determined to be 3 years old.
Based on observation, the center failed to ensure staff shall wash their hands with soap and running water after any contact with body fluids.
Evidence: Staff 4 was observed wiping a child?s nose and then immediately picking up another child. Staff 4 did not wash their hands after contact with body fluids.
Evidence: Staff 6 transported 2 children to school during the inspection. Staff 6 stated that they took the center first aid kit with them during the transportation of the children to school. During that time there was not a first aid kit at the center.
Evidence: The first aid kit did not contain the following items: scissors; adhesive tape; triangular bandages; and first aid instructional manual.
Evidence: There was not an evacuation drill practiced in 12/2022.
Evidence: The documentation of the monthly emergency evacuation drills did not contain the following information: identity of the person conducting the drill; method used for notification of the drill; number of staff participating; any special conditions simulated; problems encountered, if any; and weather conditions.
Based on record review, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the following information: allergies and intolerance to medication or any other substances, and actions to take in an emergency situation.
2. The record for Child 2 did not contain the following information: the first date of attendance.
3. The record for Child 5 did not contain the following information: allergies and intolerance to medication or any other substances, and actions to take in an emergency situation; and name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there was not an address provided for either designated person).
Based on record review, the center failed to ensure that staff records contain all of the elements required by the standards.
Evidence:
1. The record for Staff 1 did not contain the following information: job title; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the orientation required by the job position.
2. The record for Staff 2 did not contain the following information: date of employment; and documentation that two or more references as to character and reputation as well as competency were checked before employment.
3. The record for Staff 3 did not contain the following information: job title; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the orientation required by the job position.
4. The record for Staff 4 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment.
5. The record for Staff 5 did not contain the following information: job title; documentation that two or more references as to character and reputation as well as competency were checked before employment; and documentation to demonstrate that the individual possesses the orientation required by the job position.
Evidence: The attendance record for the Infant Class contained documentation that there were 6 children in care during the 03/13/2023 inspection. There were 8 children observed in care during the inspection. The attendance record did not document the arrival of each child as it occurred.
