Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 02/26/2025 and completed on 02/27/2025. The on-site inspection began at 9:30 am and ended at 11:25 am. The inspector reviewed compliance in the areas listed above. There were 40 children present and 9 staff. The inspector reviewed 5 children?s records and 5 staff records on-site on 02/26/2025.

The inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises.

The findings from the most recent inspection, dated 09/09/2024, were not posted in a conspicuous place on the premises.
Plan of Correction: Will make sure to post most current inspection documents.
Standard 8VAC20-770-60-C-2
For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee within 30 days of service.

Caregiver 4 did not have central registry search results during the inspection and has been employed for more than 8 months.
Plan of Correction: Make sure to request prior to employment and to print results and place into the staff file.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 2, who has been attending the center for over a month, did not have documentation of an immunization record on file. Child 5, who has been attending the center for 11 months, did not have documentation of an immunization record on file.
Plan of Correction: Will get immunizations before enrollment.
Standard 8VAC20-780-140-A
Physical examinations, complete by a physician, shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

Child 2, who has been attending the center for over a month, did not have a physical exam on file. Child 5, who has been attending the center for 11 months, did not have a physical exam on file.

A tuberculosis (TB) screening is required every two years from the last screening.

The TB screening for Staff 2 was overdue by 3 months.
Plan of Correction: Will get TB screening upon their expirations.

Hazardous substances, such as cleaning materials, shall be kept in a locked place using a safe locking method that prevents access by children.

The cabinet under the sink in the women?s restroom, that contained cleaning materials, was unlocked. The cleaning materials were accessible to children.
Plan of Correction: Make sure that all cleaning materials are kept locked.

Trampolines may not be used.

There was a mini trampoline in Classroom 2.

The emergency evacuations for October, November and December 2024 were not documented.
