Inspector Notes
An unannounced, on-site renewal inspection was initiated on 09/24/2025 and completed on 09/26/2025. The on-site inspection began at 8:30 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and 9 staff. The inspector reviewed 5 children?s records, 5 staff records, and 1 applicant record.
The inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
8For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee within 30 days of service.
Staff 4 did not have central registry search results during the inspection and has been employed for 15 months.
Staff members shall submit the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening.
The tuberculosis screening for Staff 1 was overdue by 4 days. The tuberculosis screening for Staff 4 was overdue by 10 months.
Child 5 (age 2 years) who was in attendance, has a prescribed emergency medication on-site. There was not a staff member that was medication administration trained on-site.
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.
The black metal cabinets in Classroom 2 and Classroom 3, which contained various cleaning materials, were not locked to prevent access by children.
Medication 1 was on the top shelf of an unlocked cabinet in Classroom 2.
OTC Skin Product 1 expired for 4 months.
The tables in Classroom 4 were not sanitized before being used for breakfast.
The center shall maintain a separate record for each child enrolled which shall contain all of the components required by the standards.
The records for Child 1, Child 4 and Child 5 did not contain all of the components required by the standards.
