Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A 60 day monitoring inspection was initiated on 5/14/21 and concluded on 5/18/21. The owner was contacted by telephone to initiate the inspection. There were 25 children present and 7 staff. The inspector emailed the owner a list of items required to complete the inspection. The Inspector reviewed 3 children's records and 3 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law in the area of administration and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and interview, the center did not ensure that each child's record contains all the required information.

Evidence:
1. The records for child 1 (date of enrollment: 4/26/21), child 2 (date of enrollment: 5/10/21) and child 3 (date of enrollment: 3/22/21) do not contain the addresses for the two designated people to call in an emergency if a parent cannot be reached.
2. The center owner confirmed that the records for child 1, child 2 and child 3 are lacking addresses for the emergency contacts.
Plan of Correction: The center responded with the following: This was an oversight with the children's registration forms. The children's parents will be contacted to obtain the addresses for the required two emergency contacts.
