Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/17/24 and completed on 10/17/24. The on-site inspection began at 10:50am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were 65 children present and 12 staff. The inspector reviewed 6 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Documentation that each child has had a physical examination by or under the direction of a physician shall be obtained before the child's attendance or within 30 days after the first day of attendance.

The record for child 3, who has been in care for 8 months, is missing documentation of a physical examination.
Plan of Correction: The center responded with the following: The center owner will email the parent today to obtain the missing physical.

Areas of the center shall be maintained in a safe condition.

In the two-year-old classroom, the rubber baseboard had become detached from an area of the wall, creating a tripping hazard.
Plan of Correction: The center responded with the following: The rubber baseboard in the two-year-old classroom was put back in place during the inspection.

Children's hands shall be washed with soap and running water after toileting.

In the two-year-old classroom, the licensing inspector observed a staff person changing a child's diaper. The staff person did not ensure that the child's hands were washed with soap and running water after the diaper change.
Plan of Correction: The center responded with the following: Center owner discussed correct hand washing and diapering procedures with the staff person on that same day. Center management will ensure all staff are retrained on hand washing and diapering procedures.

Children's records are required to contain each parent's home and employment addresses and phone numbers.

The record for child 1, who has been in care for 4 months, was missing the home address and phone number and the place of employment and work number for one parent and the work number for the other parent.
The record for child 2, who has been in care for 1 month, was missing the work number for one parent.
