During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site remenwal inspection was initiated and completed on 9/8/25. The on-site inspection began at 11:10am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 65 children present and 15 staff. The inspector reviewed 6 children's records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from one state for Staff 1, who has been employed for one week.
Plan of Correction: The center responded with the following: The out-of-state background check for staff 1 will be completed and sent today. Once received, the results will be placed in staff 1's record. Going forward, center management will ensure all out-of-state background checks are requested before hire.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for child 2, enrolled for over a year and a half, does not contain documentation of immunizations.
Plan of Correction: The center responded with the following: The parent was contacted during the inspection to request a copy of the child's immunization record. Once obtained, the documentation will be placed in the child's record.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all required information. The record for child 1, enrolled for eight months, does not contain the address of one of the emergency contacts.
