Inspector Notes

An unannounced non-mandated monitoring inspection, required by the Conditional License, was conducted on-01/26/2023. There were 5 children, ages 11 years-15 years, and 3 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, programs, and special care provisions and emergencies. A total of 5 children?s records and 4 staff record were reviewed. The children were observed during creative movement, educational lessons and during music. The inspector arrived to the center at 9:05 am and departed at 11:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the center failed to ensure that staff shall submit documentation of a negative tuberculosis screening at least every two years from the date of the initial screening or testing.

Evidence:
1. The most recent tuberculosis screening in the record for Staff 1 was date 01/05/2021; there was not a follow-up tuberculosis screening available for review.
2. The most recent tuberculosis screening in the record for Staff 2 was date 01/06/2021; there was not a follow-up tuberculosis screening available for review.
