Inspector Notes

An unannounced, on-site renewal inspection was initiated on 03/17/2025 and completed on 03/19/2025. The on-site inspection began at 9:15 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 staff. The inspector reviewed 5 children?s records, 4 staff records and 5 applicant records.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center must obtain a completed sworn statement by the end of the 30th day from taking office for new board officer/applicants.

The sworn statement for Applicant 4 was not completed by the end of the 30th day from taking office.
Plan of Correction: New Board Officers will complete the required background checks in the required timeframes.
Standard 22.1-289.036-B-2
The center must obtain a completed national criminal background check by the end of the 30th day from taking office for new board officer/applicants.

Applicant 4, who has been in office for more than 30 days, does not have a completed national criminal background check.
Plan of Correction: An appointment has been made for fingerprinting. Moving forward, new Board Officers will complete the required background checks in the required timeframes.

The center must request a search of the central registry by the end of the 30th day from taking office for new board officer/applicants.

The search of the central registry requests for Applicant 2, Applicant 3 and Applicant 4 were not made by the end of the 30th day from taking office.
Plan of Correction: The requests have been made. Moving forward, new Board Officers will complete the required background checks in the required timeframes.

A tuberculosis (TB) screening is required every two years from the date of the last screening.

The TB screening for Staff 1 was overdue by one month. The TB screening for Staff 3 was overdue by eight months.
Plan of Correction: The TB results will be added to the file upon completion.

Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Staff 2 stated that children use hand sanitizer after eating lunch rather than washing with soap and running water or a disposable wipe.
Plan of Correction: Disposable wipes will be used after lunch.
