Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on-site September 4, 2024 from 1:08pm until 1:50pm. There were 7 children present with 3 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 16,, 2024. A POC submitted after this date will not appear on the public website.

Based on a review of caregiver records and interview, the vendor failed to ensure that each caregiver complete the Virginia health and safety update course annually.
Evidence: 1. The record of caregiver #1 did not contain documentation of the 2024 Health and safety update.
2. The record of caregiver #2 contained documentation of the 2023 training dated 5/2/23. The record did not contain documentation of the 2024 training.
3. The record of caregiver #3 contained documentation of the 2023 training dated 8/10/23. The record did not contain documentation of the 2024 training.
4. The provider reported that the 2024 Health and Safety update had not been completed for the staff.
