The record of caregiver #2 did not have a subsequent sworn statement five years from the date of the previous check. The sworn statement was overdue by 159 days.

The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
1. Child #5 had been in care for over five hours and child #6 had been in care for approximately four hours at the time of inspection and had not been signed in on the attendance record.

Poisonous materials shall be stored in an area inaccessible to children. Several poisonous cleaning chemicals were in unlocked cabinets under the kitchen sink, accessible to children.

Medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.
